Why First Principles Exists

First Principles Sports Physical Therapy logo

First principles thinking means breaking a problem down to its most fundamental parts and rebuilding from there—rather than relying on assumptions or one-size-fits-all solutions.

In physical therapy, that goes beyond just identifying what hurts. It means understanding how you move, what drives your symptoms, and what actually motivates you as an individual.

At First Principles, we build your plan around the factors that matter most—both physically and personally—so the process makes sense, feels relevant, and is something you’re fully invested in.

Most Rehab is Built on Assumptions. Ours Isn’t.

Most physical therapy follows a template—treat the symptoms, apply a standard protocol, and hope it works.

At First Principles, we take a different approach. We break problems down to what’s actually driving your pain, your movement, and your limitations.

Then we build your plan around you—so it makes sense, keeps you engaged, and leads to real, lasting results.

  • A performance-focused physical therapy practice built around understanding the individual—not just the injury.

  • We identify what’s actually driving your symptoms, then build a plan that fits how you move, train, and live.

  • Active individuals and athletes who want to recover fully, move better, and get back to performing at a high level.

  • One-on-one sessions, a clear plan built around you, and a process designed to keep you engaged from start to finish—not just checking boxes.

Built For People Who Want More From Their Care

You want a provider who actually listens—someone who takes the time to understand your specific situation and builds a plan around you.

Whether you’re working to get back to training or sport, or focused on improving your overall health and performance, your care should reflect your goals—not a template.

At First Principles, no two plans look the same. We work with a wide range of active individuals and build each plan around what matters most to you—how you move, what you’re dealing with, and where you want to go.

And if we ever feel like we’re not the right fit to help you, we’ll be upfront about it and point you in the right direction.

Whether you’re dealing with a recent injury or flare-up, need to get out of pain quickly for an upcoming race or match, or you’re looking for a longer-term solution, your plan will be built around what matters most to you.

Care should feel like it was built for you—because it is.

How It Works

One-on-one consultation between physical therapist and patient discussing care plan

Start With a Consultation

A complimentary consultation (30-45 minutes) where we take time to understand your situation and, when appropriate, perform a brief hands-on assessment to begin identifying what’s going on and what the next steps should be.

Begin With an Initial Evaluation

If we’re a good fit, we’ll complete a more thorough evaluation to fully understand what’s driving your symptoms, how you move, and what needs to change. You’ll leave with a clear plan and your first treatment.

Build & Progress Your Plan

From there, we guide you through a personalized plan that evolves with you—so you continue to make progress and get real, lasting results.

Frequently Asked Questions

If you’re ready, request a consultation and we’ll take it from there.

  • No. New Jersey is a direct access state, which means you can begin physical therapy without a physician referral.

If care extends beyond 30 days or requires certain medical considerations, we’ll coordinate with your physician as needed to stay fully compliant with NJ law.

  • Yes. The consultation is completely complimentary. It’s a chance for us to understand your situation, answer your questions, and determine if we’re the right fit.

  • We are an out-of-network provider. This allows us to deliver fully individualized care without the limitations of insurance-based models.

We provide complimentary benefits verification prior to your consultation, so you have an understanding of your coverage. If you’d like your benefits reviewed ahead of time, you can send your insurance information to info@firstprinciplespt.com.

We also provide superbills for potential reimbursement. Many patients are eligible for partial to full reimbursement depending on their plan, and we’ll help you understand what that may look like before getting started.

We also accept HSA and FSA payments.

  • Every session is one-on-one with a Doctor of Physical Therapy—no aides, no handoffs, no template care.

We take a first principles approach, meaning we break your situation down to what’s actually driving your symptoms and performance limitations. From there, we build and adjust your plan in real time.

Techniques such as manual therapy, dry needling, cupping, and blood flow restriction (BFR) are not applied generically—they’re selected and integrated based on what you need that day to move forward.

  • Your initial evaluation is a continuation of the consultation, where we take a deeper look at your movement, symptoms, and goals.

We expand on what we learned during the consultation, using more in-depth assessment and testing to build a clearer picture of what’s driving your symptoms and begin making meaningful progress

You’ll leave with a clear understanding of what’s going on and how we’re going to address it.

  • We focus on measurable progress from the start.

Using a test–retest approach, we track meaningful changes each session. Most people experience some level of symptom improvement early on. When they don’t, it’s typically because that response was expected based on your presentation.

Clear communication is key—we align expectations from the beginning and adjust your plan based on how you respond.

  • We work with a wide range of active individuals—from recent injuries and flare-ups to ongoing pain or performance limitations.

Rather than focusing on a specific body part, we focus on what’s actually driving your symptoms and build a plan around you—your movement, your goals, and your timeline.

We also regularly work with individuals who haven’t gotten the results they expected from previous physical therapy, and are looking for a more individualized and thoughtful approach.

In addition, we have experience with more complex and higher-level cases, including post-operative rehab such as ACL reconstruction, where attention to detail and progression matter.
